Terms of Service (DSOA)

These Terms of Service apply to clients receiving speech pathology services funded under the Disability Support for Older Australians (DSOA) Program. They align with the DSOA Program Manual (Version 14 – June 2024) and are subject to periodic updates. The current version is available on our website.

Your Privacy

Hobart Allied Health Pty Ltd, trading as Hobart Speech Pathology, follows the Privacy Act 1988 and the Australian Privacy Principles.

For more information on how we collect, use and store your information, please read our Privacy Policy.

We will share relevant progress notes and reports with your DSOA Service Coordinator as required for ISP monitoring and review.

Cost

Hobart Speech Pathology charges service fees at the rates in the DSOA Service and Pricing Schedule (Appendix A).

Speech pathology services are provided under the support item:

Assessment Recommendation Therapy and/or Training (incl AT) – Other Therapy

    National rate: $193.99 per hour
    Regional rate: $290.99 per hour (where applicable)

Payment

Invoices are issued directly to your DSOA Service Coordinator.
Every invoice references your Individual Support Package (ISP) Number.
You are not personally liable for payment of DSOA-funded services.

Appointment Cancellation

Please provide at least 48 hours (2 business days) notice to cancel or reschedule an appointment.
Late cancellations or non-attendance may incur the full session fee.

Rights and Responsibilities

You have the right to:
•    Be involved in setting your speech pathology goals
•    Receive respectful, high-quality services
•    Provide feedback at any time

You agree to:
•    Attend appointments or give adequate notice
•    Provide a safe environment for home visits

Feedback, Compliments and Complaints

We welcome your feedback. Please speak with your speech pathologist in the first instance.

For DSOA-funded services, you should also contact your DSOA Service Coordinator, who has overall responsibility for quality and complaints.

Changing or Ending this Agreement

You or your DSOA Service Coordinator may request changes or end this agreement at any time by giving written notice. We request 28 days’ notice where possible to ensure continuity of care.

Support to Talk to Hobart Speech Pathology

If you need an interpreter, we can arrange support through the Translating and Interpreting Service (TIS National) on 131 450.
